Measles Cases Surge Globally by 20% in 2023 Amid Vaccine Gaps

…Over 107,000 Lives Lost as Vaccination Coverage Falls Short

By Bunmi Yekini

A staggering 10.3 million cases of measles were recorded worldwide in 2023, marking a 20% increase from the previous year, according to the World Health Organization (WHO) and the U.S. Centers for Disease Control and Prevention (CDC). The alarming rise is attributed to inadequate global immunization coverage, with over 22 million children missing their first dose of the measles vaccine.

“Measles vaccine has saved more lives than any other vaccine in the past 50 years,” emphasized Dr. Tedros Adhanom Ghebreyesus, WHO Director-General. “To save even more lives and stop this deadly virus from harming the most vulnerable, we must invest in immunization for every person, no matter where they live.”

The 2023 data show that just 83% of children received their first measles vaccine dose, while only 74% received the critical second dose. Public health experts warn that a 95% coverage of both doses is required to prevent outbreaks of this highly contagious virus.

A Growing Global Health Crisis

The effects of insufficient immunization are evident, with 57 countries experiencing significant measles outbreaks last year, up from 36 in 2022. Hardest hit were the African, Eastern Mediterranean, European, South-East Asia, and Western Pacific regions.

The African region alone accounted for nearly half of all large outbreaks. CDC Director Mandy Cohen underscored the importance of sustained efforts, stating, “The number of measles infections is rising around the globe, endangering lives and health. The measles vaccine is our best protection against the virus, and we must continue to invest in efforts to increase access.”

Despite the rise in cases, global deaths from measles saw a slight decrease, dropping 8% to an estimated 107,500.
This reduction is attributed to better access to health services in some regions. However, the toll remains unacceptably high, with most fatalities occurring in children under five.

Measles Elimination Goals Under Threat

The surge in cases threatens the global measles elimination target outlined in the Immunization Agenda 2030. While 82 countries achieved or maintained elimination by the end of 2023, urgent action is needed to sustain progress. Brazil’s recent re-verification of measles elimination restored the WHO Americas Region’s measles-free status, offering hope amid the setbacks.

Targeted efforts are now critical in fragile and conflict-affected settings, particularly in the African and Eastern Mediterranean regions. Strengthening routine immunization programs, bolstering disease surveillance, and delivering high-quality vaccination campaigns are essential to curbing the spread of measles.

Dr. Ghebreyesus summed up the urgency: “Without concerted action, we risk undoing decades of progress. We must act swiftly to protect every child and secure a future free of measles.”
